Low-income HIV/AIDS and Cancer Patients can apply for the Sweetleaf Patient Network. You’ll need the following:

  1. Letter of Diagnosis

  2. Current Doctor Letter of Recommendation

  3. Proof of Low Income

  4. Government-issued ID

  5. Consent forms (sent via email after submission)
