Becoming a Sweetleaf Health Equity patient

Sweetleaf Health Equity currently serves more than 150 patients. Applicants move to the active participant list when space becomes available.

Low-income HIV/AIDS and Cancer Patients can apply for the Sweetleaf Patient Network by providing the following:

1. Letter of Diagnosis

2. Current Doctor Letter of Recommendation

3. Proof of Low Income
